*TERMS AND CONDITIONS Claims will be reviewed in the order they are received. Please allow 4-6 weeks to receive your $25 rebate check. Valid in USA and Canada. Promotion begins 9/1/05 and ends 9/30/05. Claims postmarked after November 30, 2005 will not be honored. Offer good only in the U.S. (50 states, D.C., and Puerto Rico) and Canada. Offer does not apply to any remanufactured products or products purchased for resale. Keep a photocopy of the form, UPC bar code and receipt for your records. All submissions become Cooperative Media property and will not be returned. For purchases made online, please send the original packing slip or the e-mail confirmation as your receipt. Offer limited to maximum of one rebate per ignition purchased, with a maximum of two rebates per name or address. Requests from groups or post office boxes will not be honored. Rebates remitted in US dollars. If all requirements are not met, check will not be sent. Void where taxed, prohibited, or restricted by law. Proof of mailing does not constitute proof of delivery. Cooperative Media not responsible for lost, late, illegible, stolen, or incomplete requests, or postage-due, damaged, or separated mail. Incomplete or illegible submissions will not be returned. Cooperative Media agencies, distributors and resellers ineligible. Altered request materials are invalid. Cooperative Media reserves right to request additional information about a request and to refuse to honor request if not satisfied that these terms and conditions have been met, or if a submission is suspected fraudulent. Fraudulent submissions could result in prosecution under applicable laws. Rebate check will be mailed to address on rebate form. It is your responsibility to provide for mail forwarding in event of address change. Allow 4-6 weeks for processing and delivery of check. Not valid with any other Offer. Any obligation of Cooperative Media under this Offer will expire and be null and void if check not cashed or deposited within 60 days of date of issue.
